Mashup Score: 19AJKD Atlas of Renal Pathology: Acute Pyelonephritis - 9 hour(s) ago
Acute uncomplicated pyelonephritis is most commonly due to bacterial infection ascending from the lower urinary tract, and occurs more often in young adult women, 15 to 29 years old, who are sexually active; this is on account of the shorter urethra in women and its potential contamination with intestinal bacteria. As discussed in the following, complicated acute pyelonephritis has predisposing findings. The incidence is estimated to be 15 to 17 cases per 10,000 women, contrasting 3 to 4 cases per 10,000 men.

Mashup Score: 8
A 47-year-old man with acute worsening of chronic diarrhea was admitted to the hospital with a serum creatinine of 2.2mg/dL from a baseline of 1.6mg/dL. Medical history was remarkable for type 1 diabetes mellitus (diagnosed 8 years prior) complicated by kidney failure requiring kidney-pancreas transplant (cytomegalovirus [CMV] IgG-positive donor; CMV IgG-negative recipient) 3 years before presentation. The patient reported having long-standing diarrhea with 5 to 8 bowel movements per day for 25 years.

Background It is unclear how often survival benefits observed in single-center randomized controlled trials (sRCTs) involving critically ill patients are confirmed by subsequent multicenter randomized controlled trials (mRCTs). We aimed to perform a systemic literature review of sRCTs with a statistically significant mortality reduction and to evaluate whether subsequent mRCTs confirmed such reduction. Methods We searched PubMed for sRCTs published in the New England Journal of Medicine, JAMA, or Lancet, from inception until December 31, 2016. We selected studies reporting a statistically significant mortality decrease using any intervention (drug, technique, or strategy) in adult critically ill patients. We then searched for subsequent mRCTs addressing the same research question tested by the sRCT. We compared the concordance of results between sRCTs and mRCTs when any mRCT was available. We registered this systematic review in the PROSPERO International Prospective Register of System

mechanistic link between the deficiency polycystin-1 and mitochondrial homeostasis and the activation of the cyclic guanosine monophosphate-adenosine monophosphate synthase (cGAS)/stimulator of the interferon genes (STING) pathway. Our data identify cGAS as an important mediator of renal cystogenesis and suggest that its inhibition may be useful to slow down the disease progression. Background Immune cells significantly contribute to the progression of autosomal dominant polycystic kidney disease (ADPKD), the most common genetic disorder of the kidney caused by the dysregulation of the Pkd1 or Pkd2 genes. However, the mechanisms triggering the immune cells recruitment and activation are undefined. Methods Immortalized murine collecting duct cell lines were used to dissect the molecular mechanism of cyclic guanosine monophosphate-adenosine monophosphate synthase (cGAS) activation in the context of genotoxic stress induced by Pkd1 ablation. We used conditional Pkd1 and knockout cGas−/− g
