Many kidney transplant patients have glomerular disease as the cause of kidney failure. These patients may be exposed to immunosuppression before transplantation which could increase the risk for infections following receipt of a transplanted kidney. We identified kidney transplant recipients at a university teaching hospital who received immunosuppression pre-transplant for the treatment of glomerular kidney disease. We examined their risk for infection after transplantation by comparing it to the risk among transplant patients who were not exposed to immunosuppression prior to transplant. We observed no increased risk for infection following exposure to prior immunosuppression. Therefore, patients exposed to significant amounts of immunosuppression before transplantation may not require special surveillance or medication adjustment for fear of infection following their receipt of a kidney transplant.

Clinical and laboratory follow-up for living kidney donors (LKDs) may quantify risk, prompt early intervention, and inform donor candidates and donors.1 In 1999, the Organ Procurement and Transplantation Network (OPTN) requested that US transplant centers submit LKD follow-up forms 6 and 12 months after donation.2 Additional elements such as insurance status were added in 2004, with follow-up extended to 24 months in 2008. Noting poor performance, in 2013, the OPTN defined minimum transplant center follow-up thresholds, targeted to ≥80% and ≥70% for clinical and laboratory data, respectively.

Whether “sex assigned at birth“ or a binary “gender identity“ is most appropriate for the computation of estimated glomerular filtration rate (eGFR) is unknown. Furthermore, the use of gender-affirming hormone therapy (GAHT) for the development of physical changes to align TGD people with their affirmed gender is increasingly common, and may result in changes in serum creatinine and cystatin C, the biomarkers commonly used to estimate glomerular filtration rate.

A 17-year-old girl was admitted to the hospital with a 10-day history of bilateral flank pain radiating to the back, associated with low-grade fever without urinary symptoms. She had no significant medical history and denied exposure to any medications in the recent past. Kidney C5b-9 staining was performed as a marker of terminal pathway activation, intra-renal immune response was characterised through transcriptomic analysis. Results: Eighty-eight percent of biopsies showed C5b-9 deposits in glomeruli. Biopsies were grouped according to the amount of C5b-9 deposits (no or low n=15/42, 36%, intermediate n=15/42, 36%, and high n=12/42, 28%). Patients with high C5b-9 deposits significantly differed from the 2 other groups patients and were characterized by a significant higher histological chronicity score (p=0.005) and lower outcome-free survival (p=0.001). In multivariable analysis, higher glomerular C5b-9 remained associated with poor kidney prognosis after adjustment.
