The Global Burden of Disease assessment estimates that 20% of global type 2 diabetes cases are related to chronic exposure to particulate matter (PM) with a diameter of 2·5 μm or less (PM2·5). With 99% of the global population residing in areas where air pollution levels are above current WHO air quality guidelines, and increasing concern in regard to the common drivers of air pollution and climate change, there is a compelling need to understand the connection between air pollution and cardiometabolic disease, and pathways to address this preventable risk factor.

A 30-month-old girl attended our clinic for exploration of optic nerve atrophy after she initially presented with respiratory difficulties at 20 months of age. The patient, who had been born at 39 weeks of gestation, also had feeding difficulties, nasal obstruction, and a reduced ability to open her mouth; she had developed progressive macrocephaly in the first few months of life (appendix).

The purpose of this European Society for Radiotherapy and Oncology (ESTRO) project, endorsed by the European Association of Urology, is to explore expert opinion on the management of patients with oligometastatic and oligoprogressive renal cell carcinoma by means of stereotactic ablative radiotherapy (SABR) on extracranial metastases, with the aim of developing consensus recommendations for patient selection, treatment doses, and concurrent systemic therapy. A questionnaire on SABR in oligometastatic renal cell carcinoma was prepared by a core group and reviewed by a panel of ten prominent experts in the field.

The requirement of large-scale expensive cancer screening trials spanning decades creates considerable barriers to the development, commercialisation, and implementation of novel screening tests. One way to address these problems is to use surrogate endpoints for the ultimate endpoint of interest, cancer mortality, at an earlier timepoint. This Review aims to highlight the issues underlying the choice and use of surrogate endpoints for cancer screening trials, to propose criteria for when and how we might use such endpoints, and to suggest possible candidates.

The challenge lies in finding a harmonious balance between ensuring clinical effectiveness and upholding environmental responsibility. This task involves recognising that the most significant reduction in the carbon footprint of endoscopy is achieved by avoiding unnecessary procedures; addressing the use of single-use endoscopes and accessories; and extending beyond the procedural suites to include clinics, virtual care, and conferences, among other aspects of gastrointestinal practice.

Pembrolizumab monotherapy showed antitumour activity and manageable toxicity in patients with BCG-unresponsive high-risk Ta or T1 bladder cancer without carcinoma in situ and could potentially be a suitable treatment option for patients who decline or are ineligible for radical cystectomy. However, drug-free remission was achiveable for a significant subgroup of patients. On April 30, 2024, during his first visit to Chile, Rafael Mariano Grossi, Director General of International Atomic Energy Agency (IAEA; Vienna, Austria) and Chile’s Foreign Minister Alberto van Klaveren (Santiago, Chile) signed a memorandum of understanding as part of IAEA’s Nuclear Technology for Controlling Plastic Pollution (NUTEC) Plastics initiative to combat microplastic pollution in Antarctica. Grossi also met Chile’s Minister of Health Ximena Aguilera Sanhueza (Santiago, Chile) to discuss broadening cancer care in Chile via training and nuclear medicine under IAEA’s Rays of Hope initiative.
