A 28-year-old woman in her third pregnancy presented at 13 weeks’ gestation with low-grade fever, progressive fatigue, asthenia, and gross hematuria of 1 week’s duration. Her medical history included coarctation of the aorta, corrected with stent implantation, and chronic sinusitis. Home medications included aspirin 100 mg daily, amlodipine 10 mg daily, and bisoprolol 5 mg daily. She had no history of kidney disease, and had a baseline serum creatinine of 0.67 mg/dL (estimated glomerular filtration rate of 104 mL/min/1.73 m2) 1 month prior.

Obesity is common among patients with end-stage kidney disease (ESKD) and is a pervasive barrier to kidney transplantation. Patient perspectives about barriers to weight loss and patient and health care professionals’ viewpoints about optimal obesity management in ESKD are needed.

ents typically occur late in the disease course, resulting in large trials in which most participants do not contribute clinical events. Additionally, components of the endpoint are considered of equal importance; however, their clinical significance varies. For example, KFRT initiation is likely to be clinically more meaningful than GFR decline of ≥50%. In contrast, hierarchical composite endpoints (HCE) combine multiple outcomes and prioritize each patient’s most clinically relevant outcome for inclusion in analysis. In this review, we consider the use of HCEs in clinical trials of CKD progression, emphasizing the potential to combine dichotomous clinical events such as those typically utilized in CKD progression trials, with the continuous variable of GFR over time, while ranking all components according to clinical significance. We consider maraca plots to visualize overall treatment effects and the contributions of individual components, discuss the application of win odds in kidn

ialysis Fistula Maturation (HFM) Study, we determined sensitivities, specificities, and positive and negative predictive values of multiple candidate maturation criteria using the HFM Study maturation criteria as the reference. We also compared, across the maturation criteria, relationships between maturation and fistula survival using Cox proportional hazards models. Results We included 535 of the 602 HFM Study participants. The median (interquartile range) age was 57 (47–65) years, 70% were men, and 45% were Black participants. Depending on the criterion and time frame for ascertainment (3, 4, 5, 6, or 9 months), sensitivities ranged from 57% to 100%, specificities ranged from 85% to 100%, positive predictive values ranged from 88% to 100%, and negative predictive values ranged from 65% to 100%. For all criteria, areas under the curve for the 6-month (0.90–0.97 for unassisted maturation and 0.89–0.95 for overall maturation) and 9-month time frames were similar. Attainment of unassist
