A 12-year-old boy presented with chronic abdominal pain, asthma, and a few warts without other infectious history. Laboratory evaluation was notable for hypogammaglobulinemia without evidence of protein loss and poor response to pneumococcal 23 serotype polysaccharide vaccine (see Slide 4, in this article’s Online Repository at www.jaci-inpractice.org). The patient was diagnosed with common variable immunodeficiency (CVID) and started on IgG replacement. However, given warts and chronic abdominal pain, whole-exome sequencing was performed, identifying a paternally inherited heterozygous variant in NFKB1.
