The HeartMate 3 (HM3) left ventricular assist device (LVAD) has demonstrated excellent clinical outcomes; however, pump speed optimization is challenging with the available HM3 monitoring. Therefore, this study reports on clinical HM3 parameters collected with a noninvasive HM3 monitoring system (HM3 Snoopy) during echocardiographic speed ramp tests and Valsalva maneuvers.

Use of left ventricular assist device (LVAD) therapy in adults with the Heartmate 3 (HM3) device, a centrifugal, magnetically levitated continuous-flow pump, has resulted in major improvements in event-free survival,1,2,4 hemocompatibility3, and quality of life. This benefit is extended to both short- and long-term outcomes extending to 5 years post implantation. However, if these improvements will be observed in the pediatric patient population supported on LVAD remains debatable. Children continue to represent a limited but unique patient population, further complicated by a wider heterogeneity of diseases leading to need for durable support in pediatric patients, compared to adults.
