As we mark World TB Day 2024, we take this opportunity to reflect on the 2023 UN General Assembly High-Level Meeting (HLM) on the fight against tuberculosis—a milestone in the commitment towards a more coordinated, comprehensive approach to end tuberculosis globally. The UN HLM declaration on the fight against tuberculosis includes a specific pledge that all people with tuberculosis should receive a social benefits package to mitigate financial hardship. However, it is not known how this specific pledge will be realised and through which concrete actions.

With globally 6 in 10 unintended pregnancies for women aged 15–49 years ending in abortion, access to abortion is a crucial health service for women but remains inaccessible for many women. France is the first country to guarantee access to an abortion in their constitution.

On Dec 18, 2023, WHO announced the development of a guideline on the health of trans and gender diverse people with group meetings to start in February. The announcement was met with some criticism, underlining the sometimes heated public discourse on trans and gender diverse people. LGBT and gender diverse people are an underserved community disproportionally impacted by adverse health outcomes. They still too often face stigma, discrimination, and barriers to accessing health care. Mounting evidence documents higher rates of mental health conditions among these groups compared with cisgender populations.

Health disparities related to sexual orientation and gender identity exist across multiple outcomes. Scholarship has begun to evaluate whether structural stigma—ie, societal-level conditions, cultural norms, and institutional policies that constrain opportunities, resources, and wellbeing—contributes to health burdens among LGBTQ+ individuals. We conducted a comprehensive review of quantitative studies examining this hypothesis. We found 98 articles that linked objective (ie, non-self-reported) measures of structural stigma to mental (n=57), behavioural (ie, substance use; n=27; HIV/AIDS or sexually transmitted infection; n=20), and physical (n=20) health outcomes.

Together with existing evidence, our findings showed large gender-related inequalities in self-reported mental health outcomes in England. Given the existence of self-reported unmet mental health needs, we suggest that better health care system inclusivity and health-care professional training are needed, alongside broader improvements in the social and legal environment for transgender, non-binary, and gender diverse people.
