The primary function of cholesteryl ester transfer protein (CETP) is to facilitate the transfer of cholesteryl esters and triglycerides between lipoproteins. CETP inhibitors reduce the transfer of cholesterol from high-density lipoprotein (HDL) particles to low-density lipoprotein (LDL) and very low-density lipoprotein particles (VLDL). This results in elevated HDL-C levels and a decrease in LDL-C, non-HDL-C, and apolipoprotein B (ApoB) levels.1 Observational studies have demonstrated that certain reduced-function polymorphisms of CETP are associated with a decrease in major adverse cardiovascular events (MACE) and mortality.

Recent years have witnessed an increased understanding of remnant-C’s atherogenicity, highlighting its prominent role in the residual risk of cardiovascular events, even after LDL-C reduction (1, 2). Interestingly, the widespread use of LDL-C-lowering drugs has overshadowed the understanding of remnant-C as a causal CV risk factor for over 30 years.

Patients with IHB were older, more frequently female, and had a higher prevalence of cardiovascular risk factors (all p < 0.05). At discharge, IHB patients were less likely to receive optimal medical therapy. At the one-year follow-up, all-cause mortality, major bleeding, and reinfarction were significantly higher in the IHB group (all p 1; all p < 0.001).
