Black people are disproportionately affected by mass incarceration, with nearly a third of Black men facing imprisonment during their lifetime. The medical treatment that individuals receive within the criminal-legal system can either impair or advance health equity. Unfortunately, people who are incarcerated have a high burden of chronic medical illness, a shorter life expectancy, and suboptimal rates of vaccination compared with their counterparts living in the community.

Background While the broader medical community grapples with the widely accepted notion that it takes an average of 17 years for research evidence to be incorporated into clinical practice, the implementation of evidence-based interventions in carceral settings (i.e., jails and prisons) faces longer delays, exacerbating health disparities. Main body The “prison implementation penalty” describes the significant delay in and limited adoption of evidence-based healthcare practices in carceral settings. We explore the complex challenges of implementing evidence-based interventions in jails and prisons, environments where healthcare often plays a secondary role under security and discipline. At 6:00 am each morning, John parks the “methadone van” in a large community center parking lot in Woonsocket, Rhode Island. In the county with the highest opioid overdose fatality rate in the state, the CODAC Behavioral Healthcare Mobile Health Unit brings treatment to a hot spot of the rising opioid overdose epidemic. In 2007, the Drug Enforcement Agency (DEA) ceased approving these “methadone vans.” Now, after new DEA rules, this clinic-on-wheels starts patients on life-saving treatment, driving directly to the communities most in need.
