By far the most important reason for #hyponatremia in #heartfailure is poor distal nephron flux !!! ➡️Improve nephron flow (hemodynamics!) ➡️Use powerful proximal diuretics to increase distal flow (#acetazolamide) ➡️AVP antagonists almost never needed! https://t.co/Izb8WqlAbu

Replenishing potassium & magnesium stores alone actually increases serum Na in depletional hyponatremia! K & Mg go intracellular ➡️Na goes back extracellular Best K level probably ~4.5 mmol/L 3.5-4.0 mmol/L: K deficit ~ 200 mmol (!), so be aggressive! https://t.co/HYIGa5LuXK https://t.co/hwluMEPP2b

True Na depletion is very rare in #heartfailure unless there are extra-renal losses (e.g. gastro-intestinal)... ➡️Sodium avidity early on in heart failure (https://t.co/ubrnwTijl2) ➡️Interstitial sodium buffers full of Na (https://t.co/fgQlDGkL7B)

Evaluation and Management of Hyponatremia in Heart Failure - Current Heart Failure Reports - 1 year(s) ago

Purpose of Review To provide a contemporary overview of the pathophysiology, evaluation, and treatment of hyponatremia in heart failure (HF). Recent Findings Potassium and magnesium losses due to poor nutritional intake and treatment with diuretics cause an intracellular sodium shift in HF that may contribute to hyponatremia. Impaired renal blood flow leading to a lower glomerular filtration rate…

Decongestion (instead of ultrafiltration?) : Current Opinion in Cardiology - 1 year(s) ago

mbination with other diuretic classes or molecules with diuretic properties should be considered. Mineralocorticoid receptor antagonists and sodium–glucose co-transporter-2 inhibitors are disease-modifying drugs in heart failure that favourably influence prognosis early on, advocating their start as soon as possible in the absence of any compelling contraindications. Short-term upfront use of acetazolamide in adjunction to intravenous loop diuretic therapy relieves congestion faster, avoids diuretic resistance, and may shorten hospitalization length. Thiazide-like diuretics remain a good option to break diuretic resistance. Currently, ultrafiltration in AHF remains mainly reserved for patient with an inadequate response to pharmacological treatment. Summary In most patients with AHF, decongestion can be achieved effectively and safely through combination diuretic therapies. Appropriate diuretic therapy may shorten hospitalization length and improve quality of life, but has not yet prov

@VerwerftJan @FraserG51012622 @PierpaoloPel #vasodilators only work in acute #heartfailure with low output & high SVR by increasing CO. In such patients they will improve diuretic efficiency! Always the same mistake, we are after patients with LOW SBP for vasodilator R/ https://t.co/T2X6XN0os8

It would be a big mistake to think that #HFpEF is really unlikely just based on NTproBNP <125 ng/L. As we have shown @ESC_Journals, 1/3 proven ambulatory #HFpEF patients are below this cut-off and have an increased risk of #mortality & #morbidity! https://t.co/DNkbf0QSeq https://t.co/vCLCkS0GQ2
