      Development of a Sun Protection Factor for contact lenses (CL-SPF) - 2 month(s) ago

      Objective Sun Protection Factor (SPF) of sunscreen products is well recognised by consumers. This study explored how SPF could be applied to ultraviolet radiation (UV) protection from contact lenses (CL-SPF).Methods and analysis UV transmission through 15 commercially available contact lenses and three spectacle lens materials was measured with a deuterium light source and spectrophotometer. CL-SPF values were calculated using the standard in vitro method used to test and label skin products. Ray tracing was applied to two sunglass designs to assess the effect of solar angle and head orientation on light reaching the ocular surface. Cellular damage profile of human corneal and conjunctival cells across the UV range was assessed in vitro to inform an SPF equivalent for CLs.Results CLs tested fell into three categories: CL-SPF with no UV blocker=1.0–2.0 (equivalent to using no sunscreen); CL-SPF with Class 2 UV blocker=12.3–24.8 (equivalent to SPF15); and CL-SPF with Class 1 UV blocker=5

      Effectiveness and safety of the XEN45 gel stent compared to trabeculectomy in primary open-angle glaucoma: the Gold-Standard Pathway Study - 4 month(s) ago

      Aim To compare the gel stent to trabeculectomy in the Gold-Standard Pathway Study (GPS) patients with primary open-angle glaucoma (POAG).Methods In the GPS, patients with elevated intraocular pressure (IOP) poorly controlled with IOP-lowering medication were randomised (2:1, gel stent:trabeculectomy). Aggregate primary endpoint: patients (%) at month 12 achieving ≥20% IOP reduction from baseline without medication increase, clinical hypotony, vision loss to counting fingers or secondary surgical intervention (SSI). Secondary endpoints included changes in mean IOP and medication count from baseline; postoperative interventions; visual recovery; postoperative complications; and the Symptom and Health Problem Checklist (SHPC-18) questionnaire.Results Of 130 eyes with POAG treated/analysed (gel stent, n=88; trabeculectomy, n=42); 61.4% and 69.0% met the primary endpoint, respectively (p=0.394).       High-sensitivity C-reactive protein and risk of retinal artery occlusion and ischaemic stroke: a cross-cohort study - 4 month(s) ago

      Aims Previous studies have suggested an association between retinal artery occlusion (RAO) and ischaemic stroke (IS), both associated with elevated inflammatory factors. However, the role of high-sensitivity C-reactive protein (hs-CRP) in the sequential onset of these two diseases is still unclear. Based on this evidence, we evaluated the association of hs-CRP with RAO and IS. Methods We examined hs-CRP from both the large multicentre cohort study UK Biobank and Chinese Retinal Artery Occlusion study. Cox proportional hazard models were used to study the association of hs-CRP with incident RAO and IS during the long-term follow-up in the UK Biobank. Logistic regression analysis was employed to assess the cross-sectional relationship between hs-CRP with RAO and IS in the Chinese cohort.       Efficacy and Safety of Prophylactic Agents in Prevention of Retinopathy of Prematurity: A Systematic Review and Meta-analysis of Randomised Controlled rials - 5 month(s) ago

      Objective Retinopathy of prematurity (ROP) is a leading cause of preventable childhood blindness in preterm infants with low birth weight. The efficacy and safety of prophylactic agents, including vitamin A, propranolol and lipids, in reducing ROP incidence remain unclear. This systematic review and meta-analysis evaluated the effectiveness and safety of these agents in preventing ROP.Methods and Analysis A systematic search was conducted in Embase, MEDLINE and CENTRAL databases. Eight randomised controlled trials involving 1101 preterm infants were included. We assessed the incidence of ROP at any stage, severe ROP, adverse events and mortality. Subgroup analyses were performed for each prophylactic agent. Data were pooled using the inverse variance weighting method and reported as risk ratios (RRs) with 95% CI.Results No significant reduction in ROP incidence at any stage was found in the intervention groups compared with placebo (RR=0.83; 95% CI= (0.69 to 1.00); p=0.05; I²=0%).       Zoledronic acid as adjuvant therapy in neovascular age-related macular degeneration: a randomised controlled pilot study - 5 month(s) ago

      Aims To assess the feasibility of a study protocol for a randomised controlled trial of zoledronic acid (ZA) as adjuvant therapy for neovascular age-related macular degeneration (nAMD).Methods In this 1-year, randomised, double-blinded, placebo-controlled pilot study, nAMD patients were allocated 1:1 to receive intravenous ZA 5 mg or placebo at baseline and after 6 months in addition to intravitreal anti-vascular endothelial growth factor (anti-VEGF) therapy following a treat-and-extend regimen. Bevacizumab was the first-line anti-VEGF drug, but eyes with refractory nAMD were switched to aflibercept. The primary outcome was mean change in best-corrected visual acuity (BCVA).Results 40 participants enrolled in the study, with 20 allocated to each treatment group. 38 participants received both study infusions, and all participants completed the final assessment.       Investigation of choroidal vascular alterations in eyes with myopia using ultrawidefield optical coherence tomography angiography - 7 month(s) ago

      Background/aims The study aims to evaluate choroidal vascular changes in younger patients with myopia using ultrawidefield swept-source optical coherence tomography angiography (SS-OCTA).Methods Overall, 724 eyes of 362 participants (177 males, 185 females, age: 32.43±6.20 years) underwent SS-OCTA imaging (24×20 mm). The eyes were divided into normal, low myopia (LM), moderate myopia (MM), high myopia and superhigh myopia groups according to the spherical equivalent refraction (SER). Changes in choroidal vascular thickness (ChVT) and density (ChVD) in nine grids of the fundus were analysed using the latest version of the built-in analysis software.Results Axial length (AL) showed a negative correlation with SER ( r=0.822, p=0.000); ChVT and ChVD showed a negative correlation with AL (p≤0.001) in all nine grids.       Artificial intelligence chatbots as sources of patient education material for cataract surgery: ChatGPT-4 versus Google Bard - 8 month(s) ago

      Objective To conduct a head-to-head comparative analysis of cataract surgery patient education material generated by Chat Generative Pre-trained Transformer (ChatGPT-4) and Google Bard.Methods and analysis 98 frequently asked questions on cataract surgery in English were taken in November 2023 from 5 trustworthy online patient information resources. 59 of these were curated (20 augmented for clarity and 39 duplicates excluded) and categorised into 3 domains: condition (n=15), preparation for surgery (n=21) and recovery after surgery (n=23). They were formulated into input prompts with ‘prompt engineering’. Using the Patient Education Materials Assessment Tool-Printable (PEMAT-P) Auto-Scoring Form, four ophthalmologists independently graded ChatGPT-4 and Google Bard responses. The readability of responses was evaluated using a Flesch-Kincaid calculator.       Evaluation of a self-imaging OCT for remote diagnosis and monitoring of retinal diseases - 10 month(s) ago

      Objectives To evaluate the feasibility and accuracy of a portable, self-imaging optical coherence tomography (OCT) for measuring central subfield thickness (CST) and achieving diagnostic concordance for retinal lesions compared with clinic-based spectral-domain OCT (SD-OCT). Methods This comparative, cross-sectional study was conducted between August 2020 and February 2021. Two groups of adult participants were recruited: (1) a selected cohort of 160 participants with confirmed diagnosis and (2) a consecutive cohort of 315 participants recruited randomly. All participants underwent self-imaging OCT examination, as well as standard OCT examination. CST was automatically calculated for comparisons between the two OCT devices. Diagnostic concordance for retinal lesions and the success rate of self-imaging were assessed within the consecutive cohort.
