Intracerebral hemorrhage (ICH) represents roughly 10 % of all strokes, and it is associated with worse functional outcomes as compared to acute ischemic strokes (AIS).1 A systems-of-care approach for stroke management has been increasingly recognized as an important strategy for optimizing outcomes.2 As ischemic strokes are not sufficiently distinguishable from ICH without head imaging, they are treated similarly in the pre-hospital setting unless mobile stroke units are used.3 National society guidelines recommend the development of regional systems that provide initial ICH care and rapid transfer to facilities that offer neurocritical care and neurosurgical interventions when appropriate.

Stroke embolic source have an unknown origin in 30-40% of cases. Mechanical thrombectomy for acute large vessel occlusion stroke has provided us with a method to directly retrieve the thrombi from patients for analysis. By collecting stroke-causing thrombi from known sources, we can then use high-throughput RNA sequencing (RNAseq) technology to directly measure the gene expression signatures of these clots. This may allow us to identify genetic markers to predict the cause of cryptogenic embolism.
