Mashup Score: 3Duration of Atrial Fibrillation: How Much is Too Much? - 5 hour(s) ago
International guidelines categorise atrial fibrillation (AF) largely upon duration: paroxysmal (PAF; intermittent episodes lasting 12 months) or permanent (where patient and clinician agree to discontinue the pursuit of rhythm control and accept continuous AF)(1). Whilst catheter ablation (CA) of AF is an established, guideline-recommended approach to rhythm control(1), it is recognised that outcomes are worse for those with PeAF or LSPeAF, as compared with PAF(2).

Mashup Score: 6Continuous Variant Re-Classification: An Effective Tool in the Ongoing Quest to Escape Genetic Purgatory - 1 day(s) ago
Genotype, albeit to varying degrees, impacts the diagnosis, risk-stratification, and management of many genetic heart diseases (GHDs).1 Accordingly, since the start of the new millennium, genetic testing has evolved from a research-based investigational test to a class I recommended, standard of care clinical test for patients with inherited cardiac channelopathies and cardiomyopathies such as arrhythmogenic cardiomyopathy, catecholaminergic polymorphic ventricular tachycardia, dilated cardiomyopathy, hypertrophic cardiomyopathy, and long QT syndrome (LQTS).

Mashup Score: 14The frequency of gene variant reclassification and its impact on clinical management in the inherited arrhythmia clinic - 1 day(s) ago
Genetic testing in the inherited arrhythmia clinic informs risk stratification, clinical management and family screening. Periodic review of variant classification is recommended as supporting evidence accrues over time. However, there is limited reporting of real-world data on the frequency and impact of variant reclassification.

Mashup Score: 8Atrial electro-functional predictors of incident atrial fibrillation in cardiac amyloidosis - 1 day(s) ago
Atrial fibrillation (AF) is common in patients with cardiac amyloidosis (CA) and is a significant risk factor for heart failure hospitalization and thromboembolic events. Objective: to investigate the atrial electro-functional predictors of incident AF in CA.

Left atrial appendage (LAA) closure devices have been introduced recently as an alternative for oral anticoagulation (OAC) in patients with nonvalvular atrial fibrillation (AF). Because of potential overlap of ablation target areas and the previously implanted LAA closure device, as well as potential complications such as mechanical damage to the device, left atrial (LA) ablation remains a subject of debate in these patients.

Mashup Score: 16Comparison of Different Direct Oral Anticoagulant Regimens in Atrial Fibrillation Patients with High Bleeding Risk - 2 day(s) ago
The optimal dose of Direct oral anticoagulants (DOACs) to prevent ischemic stroke (IS) and systemic thromboembolism (STE) in atrial fibrillation (AF) patients with a predisposing bleeding risk remains unclear.

Mashup Score: 17A step forward towards conduction system pacing–based cardiac resynchronization therapy, albeit a small one. - 3 day(s) ago
Starting from the very first clinical reports of the deep septal pacing technique by Mafi-Rad et al. and Huang et al., it became clear that the activation of the left ventricle (LV) during pacing near the left conduction system (the method known as left bundle branch area pacing – LBBAP) results in paced QRS that, in contrast to the conventional right ventricular pacing technique, has none of the features of left bundle branch block.1;2 On the contrary, the reported LBBAP paced QRS complexes showed rapid activation over the LV and demonstrates right bundle branch block pattern.

Mashup Score: 51Spontaneous initiation of atrial fibrillation by ectopic beats originating in the pulmonary veins - 3 day(s) ago
The main conclusion of this study is that atrial fibrillation (AF)—a complex electrical phenomenon involving self-sustained diffuse activities—originates from localized sources. What lies behind this figure, as in other discoveries, is a matter of medical gap, need to understand, and perseverance following a long chain of previous scientific efforts.1

Pulsed field ablation (PFA), an emerging non–thermal energy modality for ablation procedures, is increasingly used to ablate atrial fibrillation (AF). Notably, the myocardial selectivity inherent in the electric pulse field offers a safety advantage by sparing adjacent tissues, such as the esophagus or phrenic nerve, contrasting with the potential risks associated with standard thermal energies employed in pulmonary vein isolation (PVI) procedures.
