One of the standard approaches typically involves neoadjuvant concurrent chemoradiation with carboplatin and paclitaxel followed by esophagectomy, as established by the phase III CROSS trial [1]. However, the recent phase III ESOPEC trial demonstrated improved overall survival with perioperative chemotherapy consisting of 5-fluorouracil, leucovorin, oxaliplatin and docetaxel (FLOT) compared to CROSS and established it as the new standard of care for localized and locally advanced esophageal adenocarcinoma [2].

The autonomic nervous system (ANS) is critical to atrial fibrillation (AF) pathophysiology, but it is often difficult to differentiate vagal and adrenergic AF due to complex physiological interactions.1 Early repolarization patterns (ERPs) on electrocardiograms are associated with idiopathic ventricular fibrillation, which often develops in association with increased vagal tone.2 We previously reported that ERPs were associated with increased susceptibility to AF.3 In this study, we evaluated the association between ERP and vagally-mediated AF by assessing the autonomic function using heart rate variability (HRV) and the efficacy of catheter ablation (CA).

The implantable cardioverter defibrillator (ICD) improves survival in patients at risk of sudden cardiac death (1). Current guidelines recommend ICD implantation in individuals with symptomatic heart failure (HF) and reduced left ventricular ejection fraction (HFrEF) if they have been receiving optimal guideline directed medical therapy for at least 3-6 months and who are expected to have survival greater than one year with meaningful quality of life (1). Indeed, the guidelines were carefully crafted recognizing that many cardiac patients also have significant associated comorbidities that may impact their overall survival and quality of life.
