Multiple case reports and series published during the last 70 years have shown that short-coupled premature ventricular complexes (SC-PVCs) (coupling interval <350 ms) usually precede the onset of polymorphic ventricular tachycardia/fibrillation (PVT/VF) in patients with idiopathic ventricular fibrillation (IVF). In a recent review, Belhassen and Tovia-Brodie found that PVCs with longer coupling intervals (350–450 ms) preceded the onset of VF in 15 of 86 patients (17%). Surget et al1 reviewed the data from 79 consecutive patients with IVF.

We appreciate the thorough review of our article1 by Rivarola and Scanavacca. Interest in autonomic modulation during pulmonary vein isolation (PVI) arose from the observation of atrioventricular block or “vagal effects” during ablation. This birthed the idea that PVI might inadvertently and inconsistently affect the “autonomic system” and the outcome of PVI. However, it is crucial to note that cardioneuroablation (CNA) deliberately destroys ganglionated plexi rather than accidentally doing so. Complete CNA, as performed in our study, results in vagal denervation confirmed by histology and direct vagal stimulation.

We have read the article by Chung et al1 with great interest. Their experimental findings highlight the risk of ventricular proarrhythmia after cardioneuroablation, a significant concern that dates back to the description of the technique. Although considered safe in healthy individuals,2 the vagal attenuation effect has not been specifically evaluated in patients with heart disease yet, and its impact on this particular population is therefore unknown.

Thus, the accurate evaluation of QoL is an emerging part of the assessment of AF patients2. Several tools have arisen to evaluate clinically relevant endpoints. Patient-reported outcomes (PROs) are now commonly used to evaluate QoL in clinical trials3. Additionally, patient-reported experience measures (PREMs), though different from PROs, also focus on satisfaction with care delivery as well as patient and public involvement (PPI).
