In 2023, the field of kidney transplantation faced both successes and challenges. A record 28,142 total kidney transplants were achieved in the United States, largely due to an increase in deceased donor kidney transplants (DDKTs). While the number of adult candidates listed for DDKT slightly increased, it remained below the level in 2019, with 11.4% of candidates waiting 5 years or more. Across racial and ethnic groups, Black adult candidates had the largest increase in DDKT rate in 2023, in parallel with the Organ Procurement and Transplantation Network policy to modify waiting time (implemented January 5, 2023).

The increasing availability of clinically approved genetic tests for kidney disease has spurred the growth in the use of these tests in kidney transplant practice. Neither the testing options nor the patient population where this should be deployed has been defined and its value in kidney transplant evaluation has not been demonstrated. Transplant providers may not always be aware of the limitations of genetic testing and may need guidance on comprehending test results and providing counsel, as many centers do not have easy access to a renal genetic counselor or a clinical geneticist.

Bartonella quintana (BQ) infection is rarely described to be transmitted through solid organ transplant (SOT). We report a cluster of donor-derived BQ infection and attack rate from Bartonella seropositive donors.Retrospective study of SOT recipients that received an organ from an unhoused deceased donor (UDD) in Alberta in 2022-2023. Serology testing for Bartonella was performed with Indirect Immunofluorescent Assay from UDD and recipients from UDD with positive serology. Titers ≥1:64 considered positive.

Practical applications have been identified in healthcare in general, and there is significant opportunity in transplant medicine for generative AI to simplify tasks in research, medical education as well as clinical practice. Additionally, patients stand to benefit from patient education that is more readily provided by generative AI applications.

With the increasing prevalence of marijuana use in the U.S., many deceased organ donors have a history of marijuana use, raising concerns about infectious risks to transplant recipients. We performed a multicenter retrospective cohort study in which exposed donors were those with recent marijuana use (in the prior 12 months) and unexposed donors were those with no recent marijuana use. Primary outcomes included (1) positive donor cultures for bacteria or fungi, (2) recipient infection due to bacteria or fungi within 3 months post-transplant, and (3) recipient graft failure or death within 12 months post-transplant.

Kidney allograft inflammation, mostly attributed to rejection and infection, is an important cause of graft injury and loss. Standard histopathological assessment of allograft inflammation provides limited insights into biological processes and immune landscape. Here, using Imaging Mass Cytometry (IMC) with a panel of 28 validated biomarkers, we explored the single-cell landscape of kidney allograft inflammation in 32 kidney transplant biopsies and 247 high-dimensional histopathology images of various phenotypes of allograft inflammation (antibody-mediated rejection, T cell-mediated rejection, BK nephropathy and chronic pyelonephritis).
