Telerehabilitation with a physiotherapist was non-inferior to in-person care for chronic knee pain in a randomised non-inferiority trial by Rana Hinman and colleagues. 394 adults with chronic knee pain consistent with osteoarthritis were randomly assigned to receive five in-person (n=204) or telerehabilitation (n=190) physiotherapist consultations. At 3 months, both groups had improvements in pain (co-primary outcome; mean change 2·98 [SD 2·23] for in-person care and 3·14 [1·87] for telerehabilitation; mean difference 0·16 [95% CI –0·26 to 0·57]).

Since 1940, epidemiologists, clinicians, and scientists promoted increased awareness of the crucial importance of promoting preventive strategies for minimising the risk of poor health later in life. This is particularly relevant to children and young people with chronic inflammatory diseases, including childhood-onset systemic lupus erythematosus (SLE), which are almost always associated with increased risk of comorbidity. A wealth of literature emphasises the contribution of chronic inflammation to the development of subclinical atherosclerosis, which drives the increased risk of cardiovascular disease, and many guidelines across specialties highlight the need for cardiovascular disease risk stratification and tailored management.

Two conventional synthetic DMARD tapering strategies were associated with significantly lower rates of flare-free survival compared with stable conventional synthetic DMARD treatment, and the data do not support non-inferiority. However, drug-free remission was achiveable for a significant subgroup of patients. This trial provides information on risk and benefits of different treatment strategies important for shared decision making.

Little is known about the natural course and long-term outcomes of childhood-onset Sjögren’s disease due to its low prevalence, difference in clinical presentation between children and adults, and paucity of good quality research in paediatric populations. Delays in recognition of childhood-onset Sjögren’s disease are potentially explained by the heterogeneity of clinical manifestations at disease onset1–3 and the poor performance of adult classification criteria in children.4,5 Increased awareness in health professionals and involvement of multi-specialties are needed to improve disease recognition.

The Florida Scoring System is a paediatrician-friendly tool that can be used to assist classification and long-term monitoring of suspected childhood Sjögren’s disease. The resulting stratification has important implications for clinical management, trial design, and pathobiological research. We found a highly symptomatic patient group with negative serology and diagnostic profiles, which warrants clinical attention. We further revealed that salivary gland ultrasonography can be a non-invasive alternative to minor salivary gland biopsy in children.
