Women aged ≥70 years with ovarian cancer (OC) may experience variations in care compared to best practice guidelines, and their five-year survival is significantly lower compared to women aged <70 years. The National Gynae-Oncology Registry (NGOR) is Australia's first clinical quality registry (CQR) for gynaecological cancers, measuring patterns of care using 15 OC clinical quality indicators (CQIs) reflecting evidence-based practice. This study aimed to compare adherence to CQIs for older versus younger groups with newly diagnosed OC and evaluate differences in 5-year overall survival (OS).

Sarcopenia is common in various cancer types and predicts mortality [1]. However, many studies have solely focused on the predictive value of computerized tomography skeletal muscle index as a single criterion for sarcopenia. The HandGrip Strength (HGS) measurement, a simple and quick method, is recommended by the revised European Working Group on Sarcopenia in Older People (EWGSOP2) consensus for routine sarcopenia detection [2].

Adults aged 65+ years are expected to account for 60 % of all new cancer diagnoses by 2035 [1]. In Australia, the median age of people with lung cancer is 72 years [2]. Cancer in older adults can be challenging to treat due to a limited evidence-base from under-representation in clinical trials [3], and high prevalence of comorbidities [4] that may increase treatment toxicities and be a competing cause of mortality. Older people may also have a multiplicity of geriatric syndromes including polypharmacy, cognitive impairment, malnutrition, reduced mobility, anxiety, and depression that may impact treatment delivery and/or toxicity.

Older adults with cancer face unique challenges, including complex needs and systemic barriers to care. These factors affect their medical treatment decisions and overall quality of life, while caregivers face concurrent burdens. Building on a multi-phase, patient- and community-engaged project, this survey study aimed to validate and rank research and care priorities for older adults with cancer in British Columbia, Canada generated from our earlier work.

We read with interest the article “Unsupervised learning to identify symptom clusters in older adults undergoing chemotherapy,” which offers valuable insights into the use of machine learning to stratify symptom burden in this vulnerable population [1]. The application of unsupervised clustering to patient-reported outcomes is a promising approach to understanding heterogeneity in symptom experiences. However, we wish to highlight two methodological concerns that may impact the validity and clinical significance of the study’s findings.
