A 54-year-old man with B-cell lymphoma confirmed on gastric biopsy was transferred to the NeuroICU for deteriorating arousal while hospitalized for an abdominal hemorrhage. Brain MRI revealed an enhancing cerebellar lesion, noncommunicating hydrocephalus, and absence of brainstem lesions (Figure). The patient was comatose without cough, gag, or oculocephalic reflexes.
