A 67-year-old man presented with hemifacial weakness and maculopapular rash, treated as Bell palsy. Initial CT angiogram was normal. 6 weeks later, he developed dizziness, diplopia, ataxia, and worsening rash. MRI revealed midbrain, pontine, and thalamic infarcts.
