A 70-year-old man with myelodysplastic syndrome currently undergoing erythropoiesis stimulation therapy with luspatercept presented with worsening bilateral leg weakness and urinary retention. He had bilateral patellar hyperreflexia, but absent Achilles reflexes and plantar responses bilaterally.
