MRI of the brain revealed cerebellitis (Figure 1) with negative evaluation for paraneoplastic, metabolic, and infectious etiologies. N-methyl-D-aspartate receptor (NMDAR) antibodies were positive (1:10 serum, 1:16 CSF). Treatment included pulse methylprednisolone, intravenous immunoglobulin, plasma exchange, and rituximab, with clinical improvement but long-term imaging sequelae (Figure 2).
