The cause of sudden cardiac arrest remains undefined in approximately 5% of patients, the latter receiving the diagnosis of idiopathic ventricular fibrillation (IVF). Unraveling the mechanisms underlying the arrest is a clinical need for these often-young patients who may be negatively affected by uncertainties about their prognoses, including the risk for recurrences.1 By definition, patients with IVF lack specific structural abnormalities. However, advanced imaging techniques, such as deformation imaging, can reveal early preclinical abnormalities preceding the alterations of conventional parameters, such as left ventricular ejection fraction (LVEF).

We have witnessed growing indications for cardiac implantable electronic devices (CIEDs) in arrhythmia and heart failure treatment,1 increasing longevity of patients who warrant such devices, and a correspondingly advancing age of implanted transvenous leads that allow for such therapy. It is therefore not surprising that lead-associated complications, either due to lead malfunction or due to an epidemic of CIED-related infection, and the incidence of transvenous extractions undertaken to address these issues have also escalated.

The eCOOL-AF(1), IMPACT(2) studies demonstrated upto an 83% reduction in any esophageal injury and significant reduction in AEF with EnsoETM device in patients undergoing AF ablation, resulting in FDA granting De-Novo authorization to reduce the risk of ablation-related esophageal injury; with a significant reduction in AEF rate with proactive esophageal cooling vs.
