Stevens–Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) often leads to substantial ocular surface damage.1-4 Systemic prednisolone has been previously described as a treatment for acute SJS/TEN with variable results; however, a recent study suggests that etanercept is a safe and more effective treatment with superior skin healing and lower mortality rate (8.3% vs 16.3%) when compared to treatment with prednisolone.5 Therefore, etanercept treatment may also reduce ocular complications in SJS/TEN.

A 61-year-old immunocompetent man presented with a left V1 forehead vesicular rash, eye pain, and injected conjunctiva. Despite a 2-week oral acyclovir regimen, left eye pain persisted, with facial pain and lesions returning. Examination revealed left strabismus and binocular diplopia but unremarkable magnetic resonance angiogram of the brain. Orbit magnetic resonance imaging (MRI) revealed left optic perineuritis with left inferior and medial rectus edema without any evidence of midbrain involvement either of the third nerve nuclei or third nerve bundles within the midbrain (A).

An HIV-negative 52-year-old woman presented with a leukoplakic lesion spanning her left bulbar and palpebral conjunctiva (A). Two years prior, she experienced a chemical injury to the ipsilateral eye while receiving a cosmetic facial chemical peel. Since then, she developed the irritative lesion, which was previously treated with topical lubricants and steroids without relief. Incisional biopsy revealed hyperkeratosis (B), dysplastic dyskeratotic cells with pleomorphic nuclei, intracellular bridges, high mitotic activity (C), and invasion of the basement membrane (D), confirming conjunctival squamous cell carcinoma.

The spindle-cells were vimentin/CD34-positive and SOX10/STAT6-negative (C, brown indicates positive).
