A 37-year-old highly myopic man presented with bilateral low vision (20/125), significant ocular redness, recurrent corneal epithelial defects, and photophobia for 9 months. History included systemic hydroxyurea for sickle cell disease for 4 years. Bilateral limbal stem cell deficiency (LSCD) due to hydroyxurea was observed as follows: (A) 360° superficial corneal neovascularization, conjunctival hyperemia, with whorl-like pattern late fluorescein staining (B). Anterior segment-OCT helped diagnose LSCD due to wheel-spoke distribution of thinned epithelium on mapping (C), no palisades of Vogt, limbal crypts (D, white arrow heads, en-face mode/parallel section), or clear transition between the hyporeflective corneal-hyperreflective conjunctival epithelia (D, yellow arrow heads, B-scan).
