Mashup Score: 2PREVENT CLOT Trial: Aspirin vs. Low Molecular Weight Heparin for Thromboprophylaxis After Fracture - 7 month(s) ago
12,211 patients from 21 trauma centers in the United States and Canada were included in this non-inferiority randomized trial comparing aspirin (n=6,101) and low molecular-weight heparin (n=6110). The primary outcome of interest was death by any cause at 90 days. Secondary outcomes of interest included the risk of thromboembolic events (pulmonary embolism; deep-vein thrombosis), cause-specific death, bleeding & wound complications, and the rate of infection. Aspirin was found to be non-inferior to low molecular-weight heparin with respect to the primary outcome of death by any cause. No differences in all secondary outcomes were observed between the two groups, with the exception of the rate of deep vein thrombosis (any; distal), which was higher in the aspirin group.

Mashup Score: 1
We conducted a systematic review and meta-analysis of randomized controlled trials to determine the efficacy and safety of the addition of dexmedetomidine to the usual analgesic agents (bupivacaine, levobupivacaine or ropivacaine) for femoral nerve block versus the usual femoral nerve block alone for postoperative pain control in patients undergoing total knee arthroplasty.

Mashup Score: 0
83 elderly patients with femoral neck fractures were randomized to receive a bipolar hip hemiarthroplasty via a direct anterior approach (DAA; n=40) or an anterolateral approach (ALA; n=43). The outcomes of interest included mobility measured with the Barthel Index, pain on a Visual Analog Scale (VAS), and heterotopic ossification occurrence, all measured up to 1 year post-operation. Pain scores in the first 2 weeks were significantly lower in the DAA group compared to the ALA group. While mobility was significantly better on post-operative day 1 in the DAA group, the ALA group showed superior mobility from week 6 to end of follow-up (1 year post-operation). The rate of heterotopic ossification was also significantly higher in the ALA group in the first 3 months post-operation. The results suggest that DAA may provide superior outcomes in the early post-operative phase, particularly with respect to early mobilization and pain scores. However, mobility may be better in the ALA group as

Mashup Score: 1The INSITE Trial: Intramedullary Nailing vs Sliding Hip Screw in Trochanteric Fracture Management - 11 month(s) ago
850 patients with trochanteric fractures were randomized to receive surgical fixation using an intramedullary nail (n=423, Gamma3 Stryker) or a sliding hip screw (n=427). The primary outcome of interest was health-related quality of life, measured using the Euro-QoL 5-Dimensions (EQ-5D) scale. Secondary outcomes of interest included the incidence of revision, rate of healing, adverse events,…
