The PREPARE Trial was a cluster-randomized, crossover trial part of the PREP-IT program, which compared alcohol-based anti-septic solutions of iodine povacrylex vs. chlorhexidine gluconate in operative fracture patients. THE PREPARE Trial included a total of 8485 patients who underwent fracture fixation: 1700 patients with open limb fractures and 6785 with closed lower-limb or pelvic fractures. Randomization occurred at the hospital level. The primary outcome of interest was the incidence of surgical-site infection, which included superficial incisional infections within 30 days and deep incisional or organ-space infections within 60 days post-surgery. Secondary outcomes included unplanned fracture-related reoperations and serious adverse events. The iodone povacrylex group had a lower incidence of surgical site infections in closed-fracture patients, but the rates were similar in open-fracture patients between the two groups. 1683 patients from 14 hospitals in Canada, Spain and the United States were included in this cluster-based randomized crossover trial comparing aqueous 10% povidone-iodine vs. 4% chlorhexidine gluconate for skin antisepsis in patients undergoing open fracture surgery. The primary outcome of interest was the risk of surgical site infection. The secondary outcome of interest was the rate of unplanned re-operation within 365 days. Results revealed no significant differences in all outcomes between the two aqueous-based solutions.

One thousand six hundred patients undergoing surgery for hip fractures were randomized to receive either spinal anesthesia (n=795) or general anesthesia (n=805). Pain scores using the Numerical Rating Scale (NRS) were assessed from postoperative day 1 to 3 and again at 60, 180, and 365 days. Prescription analgesic use was measured at 60, 180, and 365 days postoperatively, and satisfaction with anesthesia care using the Bauer anesthesia satisfaction questionnaire at postoperative day 3 or discharge. Pain scores were similar between the two groups from 24 hours to 365 days postoperatively. The spinal anesthesia group presented with higher rates of analgesic use at 60 days. Satisfaction did not differ between the two groups.

Forty patients aged age 18 to 25 years old with a first episode of anterior shoulder dislocation were randomized to receive arthroscopic Bankart repair (n=20) or nonoperative management (n=20). The primary outcome of interest was recurrence of instability at two year follow-up. Secondary outcomes of interest included range of motion, return to sport, Disabilities of the Arm, Shoulder and Hand (DASH) score, the Walch-Duplay score, and the Western Ontario Shoulder Instability Index (WOSI). At two year follow-up, recurrence of instability was significantly lower in the group undergoing arthroscopic Bankart repair. The Walch-Duplay score and WOSI scores were significantly better in the surgical group. Patients had a higher return to sport in the surgical group at two years follow-up. No surgical complications occurred in either group.

Thirty patients with knee osteoarthritis (OA) and undergoing a primary total knee arthroplasty (TKA) were randomized to receive a conventional jig-based TKA (n=15) or robotic-arm assisted TKA (n=15). Outcomes of interest included changes in systemic inflammatory markers (i.e. interleukin-1β [IL-1β], interleukin-6 [IL-6], tumour necrotic factor α [TNF-α], white cell count, neutrophils, lymphocytes, platelets, lactate dehydrogenase [LDH], alkaline phosphatase [ALP], cortisol levels, hemoglobin [Hb] levels, erythrocyte sedimentation rate [ESR], creatine kinase[CK], and C-reactive protein [CRP]), planned positioning of components, periarticular soft tissue injury and bone trauma using the the Macroscopic Soft Tissue Injury (MASTI) classification system, mean length of incision, mean surgical time, mean change in Hb levels, and mean knee temperature. All inflammatory markers were evaluated at 1 day, 2 days, 7 days, and 28 days post-surgery. Mean knee temperature was also measured at 6-days

Eight hundred and twenty-three (n=823) overweight/obese knee osteoarthritis patients ≥50 years or older were randomized to receive an 18-month diet & exercise program (n=414) or an attention control (n=409), delivered in a community-based setting. The primary outcome of interest was the Western Ontario and McMaster University Osteoarthritis Index (WOMAC) pain score. Secondary outcomes of interest included body weight, waist circumference, WOMAC function, 6-minute walk test, pain medication use, and Short-Form 36 (SF-36) physical and mental subscores. All outcomes were assessed upon completion of the 18-month study. Results of this study found diet & exercise to result in a higher rate of clinically meaningful WOMAC pain score reduction vs. control. Body weight, waist circumference, WOMAC function score, 6-minute walk test, and SF-36 physical subscores were also superior in the diet & exercise group,
