Mashup Score: 20A 53-Year-Old Woman With Seizure and Cavitary Lung Mass - 5 month(s) ago
A 53-year-old woman with no significant past medical history came to her local ED after a witnessed generalized tonic-clonic seizure. CT scan imaging of the head revealed a left frontal lobe hypodensity concerning for a mass with surrounding edema. In the previous month, she had undergone ambulatory evaluation for a subacute cough that had more recently become productive of bloody sputum. Outpatient chest radiography had demonstrated a new right lung opacity, shown on subsequent CT scan imaging to be a right upper lobe (RUL) consolidation.

Storytelling is a fundamental part of being human. Our earliest ancestors in the genus Homo emerged in the Great Rift Valley in Africa approximately 2 million years ago/Somewhere within that time span, probably within the last 70,000 to 100,000 years, we gained the ability to speak. Perhaps the main reason we evolved that ability was to tell each other stories, such as the one I am going to tell you about my father, Dr David Cooper, his early death at 48 years old, and how, in the years since, I’ve sought to understand both him and what happened to him.

Mashup Score: 8Management Strategies for Acute Pulmonary Embolism in the ICU - 6 month(s) ago
Acute pulmonary embolism (PE) is a common disease encountered by pulmonologists, cardiologists, and critical care physicians throughout the world. For patients with high-risk acute PE (defined by systemic hypotension) and intermediate high-risk acute PE (defined by the absence of systemic hypotension, but the presence of numerous other concerning clinical and imaging features), intensive care often is necessary. Initial management strategies should focus on optimization of right ventricle (RV) function while decisions about advanced interventions are being considered.

Mashup Score: 55Perioperative Management of Antithrombotic Therapy - 6 month(s) ago
The American College of Chest Physicians Clinical Practice Guideline on the Perioperative Management of Antithrombotic Therapy addresses 43 Patients-Interventions-Comparators-Outcomes (PICO) questions related to the perioperative management of patients who are receiving long-term oral anticoagulant or antiplatelet therapy and require an elective surgery/procedure. This guideline is separated into four broad categories, encompassing the management of patients who are receiving: (1) a vitamin K antagonist (VKA), mainly warfarin; (2) if receiving a VKA, the use of perioperative heparin bridging, typically with a low-molecular-weight heparin; (3) a direct oral anticoagulant (DOAC); and (4) an antiplatelet drug.

In this issue of CHEST, John et al1 studied the practices and impacts of postinterview communication (PIC) after the critical care medicine/pulmonary and critical care medicine (CCM/PCCM) fellowship interviews were completed by surveying both applicants and program directors (PDs) after the 2022-2023 National Resident Matching Program (NRMP) Specialty Match. We commend the authors for completing a methodologically well-designed study by addressing a question with broad, practical implications for graduate medical education, even beyond CCM/PCCM.

Mashup Score: 4Variable Practice, Variable Results - 8 month(s) ago
PIC is common among CCM/PCCM applicants and program directors. About 50% of applicants and 20% of program directors share ranking intentions via PIC. Although PIC did not impact ROL for most applicants and program directors, a minority of applicants and program directors moved programs up on their ROL after receiving PIC from the other party. Applicants have mixed perspectives on PIC value. Applicants and program directors alike desire clear guidance on PIC to minimize ambiguous and misleading communication.
