Aims Acinar cystic transformation (ACT) of the pancreas is a rare pancreatic cystic lesion. Owing to its rarity, comprehensive histomolecular characterisation of this entity is still lacking. We aim to perform a systematic review on this controversial entity. Methods We searched PubMed, SCOPUS and Embase through May 2023 to identify all studies on ACTs. Clinicopathological, immunohistochemical (IHC) and molecular data have been extracted and analysed. Results Overall, there were 121 cases of ACTs in the literature. ACT had a female predominance (65.3% of patients), and a mean size of 4.8 cm. ACT was more often unifocal (71.9%) and multiloculate (61.2%). Histologically, the cysts were lined by an acinar epithelium, sometimes harbouring ductal-like areas (18.2%). In five cases (4.1%), an intralesional pancreatic intraepithelial neoplasia (PanIN) was reported. Preoperative diagnosis is challenging. A 57-year-old man presented with prominent lymph nodes in the neck. Cross-sectional imaging demonstrated enlarged cervical lymph nodes, a unilateral nasal cavity tumour with extension into the ethmoid and maxillary sinuses, and widespread bone metastases. A biopsy of the nasal cavity tumour showed a poorly differentiated basaloid malignancy arranged in islands and strands (figure 1). A broad immunohistochemistry (IHC) panel was performed which demonstrated variable expression of SOX10 (figure 2) together with the expression of p16, CD56 and focally synaptophysin. The Ki67 labelling index approached 100%. No expression of AE1/AE3, CK5/6, CK7, CK20, 34βE12, p63, TTF-1, CD99, S100 or chromogranin was identified. INI-1 expression was retained. The poorly differentiated morphology and SOX10 expression led to an initial diagnosis of metastatic malignant melanoma by the head and neck pathology team. EWSR1 is the most commonly rearranged gene in mesenchymal neoplasia, and its myriad chimeric oncoproteins drive widely disparate neoplasms. Here, we survey selected EWSR1 rearrangements, including well-described EWSR1 fusions with CREB family members, ATF1 and CREB1 , as well as fusions in emerging entities such as mesenchymal neoplasms with EWSR1::PATZ1 and EWSR1::NFATC2 fusions. We also discuss recent data demonstrating the imperfect specificity of EWSR1::WT1 and, possibly, EWSR1::FLI1 fusions.
