Originally, the purse-string suture (PSS) was used for lesion closure during EMR. Here, we evaluated the effectiveness of the endoscopic PSS in controlling refractory bleeding.

Although on the surface, the endoscopic evaluation of the pouch itself sounds simple, in reality it is a somewhat complex endeavor, and there is a great deal to see, think about, and analyze during a so-called pouchoscopy. There are different types of pouch, different patterns of injury and inflammation in pouches, and various postsurgical adverse events to be on the lookout for, among other factors.

A 70-year-old woman with Lynch syndrome who had previously undergone subtotal colectomy and had refractory pyloric stenosis underwent placement of a lumen-apposing metal stent (LAMS) across the stenosis (A), which was planned for removal in 12 weeks. After 10 weeks, she presented with abdominal pain and vomiting shortly after ingesting a newly prescribed vitamin B12 pill. CT revealed LAMS migration to the distal small bowel with upstream dilatation (B). Colonoscopy was unable to reach the LAMS despite deep ileal intubation.
