A 72-year-old woman presented with heartburn and difficulty in defecating for 1 month. Physical examination showed no abdominal tenderness and guarding. A gastroscopy revealed chronic atrophic gastritis, and a colonoscopy detected an Ascaris lumbricoides wriggling in the appendicular orifice (A). This parasite was successfully removed by the use of biopsy forceps (B) and measured approximately 15 cm in length (C).

Obtaining adequate tissue samples in subepithelial lesions (SELs) remains challenging. Several biopsy techniques are available, but a systematic review including all available techniques to obtain a histological diagnosis of SEL is lacking. The aim of this study is to evaluate the diagnostic yield and complication rates of endoscopic biopsies, endoscopic ultrasound guided-FNA and –FNB (EUS-FNA/B), and mucosal incision assisted biopsy (MIAB) for SELs in the upper gastro-intestinal tract.

A 31-year-old man presented with acute cholecystitis and a history of metastatic alveolar rhabdomyosarcoma. He had previously undergone left orbital exenteration and facial radiation. Given the patient’s malnutrition, he was deemed a nonsurgical candidate. The patient chose to pursue EUS-guided gallbladder drainage. Owing to radiation-induced trismus, he was not able to open his mouth more than 5 mm. Bougie dilatation of an existing gastrostomy was considered; however, there was concern about poor healing and gastrostomy leak.

Endoscopic mucosal resection (EMR) with use of electrocautery (conventional EMR) has historically been used to remove large duodenal adenomas; however, use of electrocautery can predispose to adverse events including delayed bleeding and perforation. Cold snare EMR (cs-EMR) has been shown to be safe and effective for removal of colon polyps, but data regarding its use in the duodenum are limited. The aim of this study was to evaluate the efficacy and safety of cs-EMR for nonampullary duodenal adenomas ≥1 cm.
