This document was developed using the Grading of Recommendations Assessment, Development, and Evaluation framework and serves as an update to the 2014 ASGE guideline on the role of endoscopy in the management of GERD. This updated guideline addresses the indications for endoscopy in patients with GERD as well as in the emerging population of patients who develop GERD after sleeve gastrectomy or peroral endoscopic myotomy.

A 70-year-old woman was referred to our unit because of an incidentally discovered pelvic lesion accompanied by enlargement of multiple lymph nodes and small amount of ascites (A). EUS confirmed a 4.0 × 2.5-cm mixed-echoic mass in the right ovary. We conducted EUS-guided through-the-needle biopsy sampling (EUS-TTNB) for histologic diagnosis. The EUS probe was advanced to about 10 cm from the anal verge and rotated to the right to find the ovarian mass. Under EUS guidance, a transrectal puncture was performed using a 19-gauge FNA needle, followed by the advancement of a microforceps (Disposable micro-forceps, Ushare Medical, Mentor, China) through the needle into the lesion (B, Video 1, available online at www.giejournal.org).

Endoscopy plays a key role in diagnosis, monitoring of disease activity, assessment of treatment response, dysplasia surveillance, postoperative evaluation, and interventional therapy for patients with inflammatory bowel disease (IBD). Clinical practice patterns in the endoscopic management of IBD vary. A panel of experts consisting of IBD specialists, endoscopists, and GI pathologists participated in virtual conferences and developed this modified Delphi-based consensus document to address endoscopic aspects of IBD management.

Atrioesophageal fistulas and esophagopericardial fistulas are life-threatening adverse events that are believed to progress from early esophageal mucosal injury (EI). EUS has been proposed as a superior method to EGD to survey EI and damage to deeper structures. We evaluated the safety of EUS in categorizing postablation EI and quantified EUS-detected lesions and their correlation with injury severity and clinical course.

Physical examination revealed abdominal distention, mild tenderness in the lower abdomen, and hypoactive bowel sounds. Abdominal CT revealed a 4-cm mass in the sigmoid colon, causing lumen stenosis; the adjacent mesentery and omentum exhibited nodular hyperplasia and swelling, with invasive involvement around the left ureter (A). Advanced colon cancer was highly suspected, and an emergency transverse colostomy was performed.
