A 37-year-old woman with Crohn’s ileitis and a persistent anorectal stricture presented for colonoscopy. During the procedure, a gastroscope was unable to traverse the anorectal stricture (A). Functional lumen imaging probe (EndoFlip Medtronic, Crospon Inc., Minneapolis, Minn, USA) procedure was performed to further evaluate the stricture. An 8-cm catheter was inserted into the rectum under endoscopic visualization. The stricture measured approximately 2 cm in length on EndoFlip. The balloon was inflated to 40 mL with a diameter of 9.2 mm and distensibility of 1.3 mm2/mm Hg and with minimal change in diameter (9.3 mm) and distensibility (1.4 mm2/mm Hg) at 50 mL, characteristic of a stricture (B).

This type of careful evaluation is critical before embarking on endoscopic submucosal dissection (ESD) for resection, because it can give important information as to which lesions may or may not be endoscopically resectable.

A 51-year-old woman was referred to our hospital because of an ileocecal neoplasm. The patient had undergone a salpingo-oophorectomy for endometriosis 4 years earlier. MRI of the ileocecal region showed no significant findings (A). Magnified narrow-band imaging revealed regular ducts and no vessel pattern in the lesion (B). EUS showed a hypoechoic mass from the appendiceal lumen at the orifice, 8.2 mm × 6.1 mm, with a central hyperechoic nodule. After informed consent was obtained, endoscopic transcecal appendectomy (ETA) was performed.
