My Metformin expectation? ~50% non-statistical ⬇️in ER visits / hospitals due to low ~1% event rate. We tried to combat this by ⬆️sample size to 3000. Effect would need to be 1%➡️.33% for statistical difference. Acute symptom resolution does not differ https://t.co/0GJ0c9xNxb https://t.co/vf0R6BQKCW

Objectives Little is routinely disclosed about the costs of the pivotal clinical trials that provide the key scientific evidence of the treatment benefits of new therapeutic agents. We expand our earlier research to examine why the estimated costs may vary 100-fold. Design A cross-sectional study of the estimated costs of the pivotal clinical trials supporting the approval of 101 new therapeutic agents approved by the US Food and Drug Administration from 2015 to 2017. Methods We licensed a software tool used by the pharmaceutical industry to estimate the likely costs of clinical trials to be conducted by contract research organisations. For each trial we collected 52 study characteristics. Linear regression was used to assess the most important factors affecting costs. Primary and secondary outcome measures The mean and 95% CI of 225 pivotal clinical trials using varying assumptions. We also assessed median estimated costs per patient, per clinic visit and per drug. Results Measured as

ACTIV-6 was a small part of the $9.6B in US gov research response from 2020-24. The cost was ~$13k/participant enrolled of which 40% went to institutional overhead, so more like $8k/volunteer in actual direct costs. Compared this to 2015-2017 RCT costs: https://t.co/V4CK2UHwuI https://t.co/Bw6eC3LTnp

At what point does transactional policy = bribery? Give me $1Billion, and I will repeal US environmental policy and seek to move the US auto industry backwards in a global marketplace. What is good for the country doesn't matter. It's transactional. https://t.co/o0wMLwVFz5
