It has been nearly 20 years since Onishi et al published the pivotal analysis showing that early-stage non-small cell lung cancers (ES-NSCLC) receiving a biologically effective dose (BED)10 ≥100 Gy at the isocenter had improved local control (LC).1 Prospective lung stereotactic body radiation therapy (SBRT) trials have demonstrated a 5-year LC of 90% to 96% in ES-NSCLC.2,3 However, retrospective series with longer follow-up indicate that the risk of local recurrence may be higher and may be related to how lung SBRT is prescribed.
