The diagnosis of fungal keratitis using potassium hydroxide (KOH) smears of corneal scrapings enables initiation of the correct antimicrobial therapy at the point-of-care but requires time-consuming manual examination and expertise. This study evaluates the efficacy of a deep learning framework, dual stream multiple instance learning (DSMIL), in automating the analysis of whole slide imaging (WSI) of KOH smears for rapid and accurate detection of fungal infections.

A 59-year-old woman with stage IV cutaneous melanoma, metastatic to multiple sites, presented with floaters in her left eye (A) with visual acuity of 20/50 and preretinal deposits on OCT (B). Her condition was controlled systemically with nivolumab therapy. Diagnostic pars plana vitrectomy confirmed vitreous metastasis from cutaneous melanoma cytologically. Treatment with intravitreal injection of melphalan (10-20 μg/0.1 ml) monthly for a total of 3 planned sessions resulted in tumor regression (C), observed on OCT (D), and with visual acuity of 20/60.

A 9-year-old boy presented twice in our practice with recurrent metallic foreign bodies (FB) in his left eye ([A] FB at the limbal 4 o’clock position; [B] FB at the 4 and 9 o’clock position 5 weeks later). His mother reported previous FB, which she had removed herself. Medical history and specific questioning revealed the metallic suspension of his indoor hanging swing as the source of metal wear (C, abraded karabiner and mounting). After removal of the swing, no further complaints occured. This case highlights the need for a careful review of the history in such situations.
