A 70-year-old man presented with decreased visual acuity (3/60), elevated intraocular pressure, and near-total neuroretinal rim thinning (A). Angulated blood vessels at the optic disc margin indirectly suggested a deep excavation, and this was visualized with in vivo imaging using OCT. The excavation was more pronounced in the vertical meridian, resembling “bean-pot cupping” (B), and in the horizontal meridian, resembling a “lotus petal” (C). Central retinal vessels traversing through deep optic disc cupping were also visualized (A, B, arrow).
