Metabolic acidosis is common in acutely unwell patients and indicates an underlying pathophysiological disturbance. Treatment should be primarily directed at the underlying condition, not the acidosis. Thus, a key element of managing metabolic acidosis is determining the underlying cause. In most cases, metabolic acidosis is caused by increased plasma concentrations of pathologic anions and results in an increased anion gap. However, metabolic acidosis can occur in the presence of a normal or low anion gap.

Given these numbers, even small improvements in care at an individual level may translate into significant benefits at a population level.
