Heart failure is a chronic condition caused by the heart not functioning as it should or a problem with its structure. Heart failure is on the rise with nearly 800,000 people in Canada currently living with the condition. Heart Failure Awareness Week is May 4th-10th, 2025 and this year’s focus is on navigating the symptoms of heart failure and empowering action. The webinar aims to equip primary care providers and individuals with lived experience with valuable insights into the symptoms of heart failure, the challenges associated with its diagnosis, and the steps being taken to support unified heart failure care strategies by organizations across Canada.
