In this issue of Surgery, Rhaiem et al characterized the association of R0 or R1 margins with outcomes for patients undergoing hepatectomy for KRAS wild-type (KRAS-wt) and mutated (KRAS-mut) colorectal liver metastases (CLM) between 2010 to 2015.1 Among 593 patients included in this retrospective study from 9 institutions across multiple countries, 71% were KRAS-wt. There were no differences in most patient characteristics based on KRAS status. R1 resection, defined as the presence of tumor cells within 1 mm of the resection margin or the presence of microscopic residual disease, was reported in 32.7% of patients overall, with no difference across KRAS status.

Fluorodeoxyglucose uptake on positron emission tomography imaging has been shown to be an independent risk factor for malignancy in thyroid nodules. More recently, a new positron emission tomography radiotracer—Gallium-68 DOTATATE—has gained popularity as a sensitive method to detect neuroendocrine tumors. With greater availability of this imaging, incidental Gallium-68 DOTATATE uptake in the thyroid gland has increased. It is unclear whether current guideline-directed management of thyroid nodules remains appropriate in those that are Gallium-68 DOTATATE avid.

Academic surgery is the best career one could ever aspire to have; however, given the long duration of training and the anticipated education debt, surgeon compensation has not kept pace with the compensation of other comparable careers. As surgeon compensation has experienced increased downward pressure, it has become of growing importance to those in academic medicine/surgery. Competitive compensation is necessary, even if not sufficient, for successful faculty recruitment and retention. The optimal compensation system should encourage the best possible patient care, inspire teamwork, maximize the department’s or physician practice’s ability to recruit and retain faculty, support all missions, and be viewed as equitable and transparent.

After the publication of multiple trials testing antibiotics versus appendectomy, guidelines now include antibiotic therapy as an option for patients with acute, uncomplicated (ie, localized) appendicitis.1 Nonoperative management avoids short-term risks and morbidity associated with surgery but entails a longer course of antibiotic therapy, and, significantly, a large share of patients (∼30%) will fail nonoperative management, requiring appendectomy due to recurrent appendicitis.2 When presented with both options, ∼10% to 30% of patients/caregivers will choose nonoperative management.
