A 65-year-old patient presented, 9 months after Ahmed Glaucoma Valve (AGV) implantation, with diminution of vision in the right eye. Examination revealed ballooning of the AGV bleb which was cystic with transillumination (A). The AGV plate (B, black arrow) was freely mobile in the cyst, with the tube (white arrow) having dislodged from the anterior chamber and lying horizontally within the cyst. Dislodgement of tube from the anterior chamber and epithelialization of its fistulous tract had resulted in free filtration leading to persistent hypotony.

A 50-year-old man presented with pain, an elevated intraocular pressure of 55 mmHg, and an inverse hypopyon bisecting the pupil (A). He underwent silicon oil tamponade for postcataract surgery retinal detachment elsewhere. Indirect gonioscopy revealed oil droplets in the angles and behind the iris (B). The emulsified silicon oil may have found its way through loose zonules and spilled beyond its intended boundaries, causing trabecular meshwork occlusion and glaucoma. This clinical image emphasizes the necessity of thoroughly removing silicon oil from the posterior chamber as well as the anterior chamber and vitreous cavity.

A 72-year-old woman with a history of multiple retinal detachment surgeries including silicone oil and cataract surgery in the right eye was referred for persistent hypotony.
