For decades, prostate cancer (PCa) diagnosis has depended on transrectal ultrasonography (TRUS) biopsy. The evolutionary journey of prostate biopsy began in the 1920s with the introduction of the first finger-guided transperineal (TP) needle biopsy and the open perineal prostate biopsy. In 1937, Astraldi conducted the first finger-guided transrectal (TR) prostate biopsy. The 1960s saw further advancements with the successful imaging of the prostate using a TRUS probe. In 1981, Holm et al described the first TRUS-guided TP prostate biopsy, and in 1989, Hodge et al introduced the first sextant TRUS-guided TR prostate biopsy, marking the start of the modern era of systematic biopsy for PCa.

One such area is in prostate cancer screening in transgender women (assigned male sex at birth but identify as women). A key fact that is not universally appreciated is that the prostate is not removed in any form of gender-affirmation surgery; thus, transgender women remain at risk of prostate cancer.

At the 2024 Annual Urology Advocacy Summit in Washington, DC, the Sexual Medicine Society of North America held a session focused on the health and psychological benefits of urological prosthetics in prostate cancer survivors. Dr Hossein Sadeghi-Nejad moderated the session, commencing with a poignant quote by Dr Litwin and colleagues, stressing the need to enhance not only survival but also the quality of life for men with localized prostate cancer. Highlighting the paper from their own group, and specifically looking at the SEER database between 2006 and 2011, it was demonstrated that of 13,812 patients diagnosed with erectile dysfunction in a cohort of more than 30,000 radical prostatectomy and radiation therapy patients, only 3.6% of the radical prostatectomy group and 1.4% of the radiation group underwent penile prosthesis surgery as treatment for erectile dysfunction, illustrating likely underutilization of this treatment modality.

As many as 20% to 25% of patients with metastatic prostate cancer harbor a mutation or aberration in a gene involved in the DNA damage repair (DDR) pathway. The most commonly mutated genes are BRCA1 and BRCA2, encoding proteins critical for homologous recombination repair (HRR). Patients with germline BRCA1/2 mutations are known to be at higher risk of cancer diagnosis and require frequent screening for various malignancies, and men with prostate adenocarcinoma and either somatic or germline BRCA1/2 mutations have a more aggressive phenotype.
