Urological conditions affect millions of men and women, particularly as they age, leading to challenges in urination, continence, sexual dysfunction, and reduction in quality of life. Seventy-five years of research funding in benign urology from the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) at NIH not only has greatly advanced our knowledge of etiology and mechanisms of urological conditions but has also advanced research on new technologies and treatments to improve clinical care. To illustrate the profound impact of NIDDK research and development funding on clinical care, I undertook a survey of grants funded by NIDDK in the NIH Reporter website with a focus on translation of lower urinary tract health and disease research to the clinic. Symptoms of Lower Urinary Tract Dysfunction Research Network (LURN) was established by the NIH/NIDDK in 2012 to study lower urinary tract disorders. There are 2 LURN studies. LURN I (the first phase of the LURN Study) studied lower urinary tract symptoms (LUTS) in men and women. LURN II (the second phase of the LURN Study) studied urinary urgency, with or without urgency urinary incontinence (UUI), in men and women (ie, with overactive bladder). In this article, we highlighted some of the findings from the LURN studies. This presentation was given at the 2025 Society of Urodynamics, Female Pelvic Medicine & Urogenital Reconstruction meeting celebrating the 75th anniversary of the NIDDK.

Engaging patients and caregivers in clinical trial ideation and design can help ensure these trials are feasible, impactful, and will optimize recruitment and retention. Engaging frontline clinicians in implementation research will help to understand gaps in care and utilization of care guidelines derived from these high-impact clinical trials. Urinary stones are pathological biominerals that form in supersaturated urine. While urine is typically supersaturated with crystallizing compounds such as calcium oxalate (CaOx) monohydrate, multiple mechanisms influence crystal development. Once formed, crystals can enlarge, aggregate, and adhere to tubular epithelial cells, resulting in stone development. The gut microbiome (GM) can influence lithogenesis through oxalate metabolism, changes in urinary pH, and uric acid production. Additionally, the urinary microbiome, or urobiome, may influence crystal formation directly in the kidney. One established etiological pathway of bacterial-induced stones is the development of struvite stones, in which urease-producing bacteria increase pH and reduce phosphate solubility. However, metabolic stones, characterized as the outcome of metabolic disturbances, may also be directly influenced by the urobiome.

Our management and definition of clinically significant prostate cancer have evolved over the past 2 decades. At 15 years, survival rates are similar between primary surveillance and radical treatment for prostate cancer, furthering the questions, “Who benefits from treatment?” and “How best to treat?” Evidence supporting the efficacy of focal ablation for primary treatment of prostate cancer has dramatically expanded, and practice patterns are shifting to be more inclusive of these modalities. In the salvage setting, a similar paradigm shift is emerging in the literature as well.

Benign prostatic hyperplasia (BPH) is the most common urological condition, often associated with lower urinary tract symptoms impacting quality of life. It is estimated to affect approximately 50% of men over the age of 50 and more than 85% of men in their 80s. Currently, we have a growing arsenal of surgical treatment options, ranging from minimally invasive surgical therapies to enucleation techniques. This wide availability allows us to address the needs of each patient in an individualized manner, providing increasingly personalized treatments. The first description of endoscopic enucleation of prostate (EEP) dates back to 1983, when Hiraoka published data on his technique in Japan. However, it was only after 1998, with the development of the first morcellators associated with the use of the holmium laser by Fraundorfer and Gilling, that holmium laser enucleation of the prostate (HoLEP) gained widespread popularity, marking the beginning of the modern era of EEP.
