Immune checkpoint inhibitors (ICIs) have substantially advanced the treatment landscape for a wide variety of malignancies. Older adults represent a large and rapidly growing demographic, among whom ICIs are widely prescribed. Management of ICI-associated toxicity among older adults, particularly in the presence of frailty and comorbidity, poses unique challenges. In this Policy Review, developed by the International Society of Geriatric Oncology (SIOG), we offer an evidence-based framework for health-care providers, caregivers, and policy makers for treating older adults with ICIs, focusing on unique considerations for this population that are not adequately addressed by existing guidelines, and expanding them to encompass geriatric oncology principles.

The global incidence of cancer reached 20 million in 2022 and is projected to rise to 33·1 million by 2050.1 Amid the growing cancer epidemic, the ability to provide appropriate care for patients is compromised not only by inadequate infrastructure and treatment options, but also by the paucity of needed personnel. Latin America and the Caribbean, which comprise 33 countries—44% of them low or middle income—are known to have large oncology workforce shortages. For example, according to an analysis of Global Cancer Observatory data, Brazil had only 646 radiation oncologists and 533 medical physicists in 2022, leaving deficits of 960 and 359, respectively, compared with estimated needs; together, these deficits constituted the second-highest gap in radiotherapy professionals among 32 countries of varying income levels included in the report.

However, questions regarding the importance of subsequent treatment administered to trial patients in the control group (often placebo or surveillance) after recurrence quickly arose, leading to debates over trial design and application in real-world practice.
