Hidradenitis suppurativa (HS) is a chronic, inflammatory condition predominantly affecting younger adults and disproportionately affecting women and Black Americans. Estimated prevalences vary significantly according to study methodology; however, true prevalence may range from approximately 0.1% to 1%. Hidradenitis suppurativa is associated with a number of comorbid conditions which also contribute to poor health outcomes and negative impact on quality of life. Recent advancements in our understanding of disease burden in hidradenitis suppurativa have allowed us to identify subgroups of the population afflicted with the disease and to implement strategies which facilitate comprehensive care.

Hidradenitis suppurativa is a debilitating inflammatory disease with a profound impact on quality of life for affected patients and caregivers. The patient journey in hidradenitis begins with symptom onset and proceeds through diagnosis and treatment. It is fraught with misdiagnosis, lack of awareness, misinformation, and inappropriate treatments that lead to shame, blame, isolation, and medical mistrust. This review highlights the relationship between hidradenitis and quality of life, emphasizing the patient journey and opportunities to improve care along its tortuous course.

Hair loss is devastating to the patient, but the diagnosis and treatment of alopecia are also difficult for primary care providers and even dermatologists. Given recent advances in the field, there has never been a more pressing time to reevaluate and improve upon the significant clinical skills needed to accurately diagnose and treat our patients with hair loss. This CME activity on alopecia is designed to (1) educate dermatologists in a simple, succinct fashion on the identification of, and the distinction between, the diverse types of hair loss and (2) discuss how to manage them.

“HS is the worst disease in dermatology.” As dermatologists specializing in hidradenitis suppurativa (HS) care, we have heard this sentiment many times from both patients and clinicians. We have witnessed HS derailing a patient’s life trajectory since adolescence and robbing individuals of their self-esteem, ability to function, and career and family planning dreams. On the clinician’s side, we understand the frustration of feeling uncertain about the best next steps in treatment when options are limited, and our best efforts are not doing enough to ease a patient’s suffering.

Pachyonychia congenita is a group of rare keratinization disorders primarily characterized by the triad of palmoplantar keratoderma, nail hypertrophy, and plantar pain (Figs 1-8).1 Estimated prevalence is 5000 to 10,000 patients worldwide, though true prevalence is likely higher due to underdiagnosis of milder subtypes.2 Plantar pain is often debilitating and significantly affects quality of life.3,4 Most cases are inherited in an autosomal dominant fashion, though around 30% are due to de novo mutations.

To the Editor: Vancomycin accounts for a sizable portion of drug-induced hypersensitivity syndrome (DiHS)/drug reaction with eosinophilia and systemic symptoms (DRESS)1 and is associated with 10% to 40% higher rate of organ involvement than overall DiHS/DRESS.2 Research is lacking describing the characteristics of vancomycin associated DiHS/DRESS specifically and the etiology behind extensive organ involvement in these cases. The purpose of this study was to describe vancomycin associated DiHS/DRESS and investigate clinical variables related to organ involvement.

Antibody-drug conjugates (ADCs) are an emerging class of anticancer agents that combine targeting antibodies with potent cytotoxic agents. Their molecular configuration allows for increased therapeutic efficacy and reduced adverse-effect profiles compared to monoclonal antibodies or cytotoxic chemotherapy alone. ADCs cause off-target toxicities through several mechanisms, including premature deconjugation of the cytotoxic agent in the serum and the presence of the targeted antigen on normal tissues.
