Anorexia nervosa (AN) is a severe mental illness with substantial morbidity and mortality. The central, salient disturbance in AN is restriction of food intake, leading to inappropriately low body weight. Onset of illness is most common during mid-adolescence, and approximately 1% of female individuals are affected over a lifetime, across all socioeconomic classes.1 Despite advancements in treatment for adolescents with AN, remission rates remain disappointing—less than 50% of teens typically respond to initial treatment.2 Among those who achieve remission, subsequent relapses of AN are common, as is the presence of affective disorders later in life.2

Humans are hardwired for connection. Feeling as though you are tethered to another person, community, or movement breathes meaning into life. Whether you are an infant or more seasoned, the desire to connect to an entity outside of yourself carries tremendous weight. As child and adolescent psychiatrists, we are trained to celebrate and reinforce the positive connections in a child’s life, while recognizing and helping to prune away unhealthy ones. While watching my timid 7-year-old son find his way on an intimidating all-star baseball team, I am witnessing the gravity of connection right in front of me.

Julian, a 16-year-old adolescent who entered the juvenile justice system facing armed robbery charges, was 8 years old when he entered the child welfare system. He and his younger sister, Jessica, were placed in protective custody after school staff discovered that they were physically abused and witnessed domestic violence. Jessica further disclosed sexual abuse by the mother’s boyfriend who was subsequently incarcerated. The siblings were placed together in several foster homes until a biological aunt assumed custody for 2 years. Both children struggled with behavioral problems in school and at home and responded to mental health treatment. The mother participated in substance use treatment, and the children reunified with her by the time Julian was 11 years old.

Since the 1890s, scholars such as W.E.B. DuBois have understood that Black people living under conditions of pervasive White supremacist oppression are subjected to constant assaults on their self-image.1 In the intervening years, Fanon,2 Morrison,3 and many others have described the complex psychological consequences of Black Americans attempting to live and adapt to the dominant culture, which devalues Black lives and Black cultural traditions, and how these devaluing messages reaffirm their inferior status in society.
