Around 25 000 people die from tuberculosis every week, despite the disease being curable. Progress in reducing tuberculosis morbidity and mortality is hindered by inadequate access to testing and treatment, with at least 40% of those infected remaining undiagnosed. Our 2022 Series, highlighted the huge scale of the problem, exacerbated by COVID-19, and outlined priorities for rapid restoration of tuberculosis services. In a 2023 Editorial, we discussed the need for substantial investment in vaccines and research to improve health-care services in LMICs.

Drug-resistant tuberculosis (DR-TB) threatens to derail tuberculosis control efforts, particularly in Africa where the disease remains out of control. The dogma that DR-TB epidemics are fueled by unchecked rates of acquired resistance in inadequately treated or non-adherent individuals is no longer valid in most high DR-TB burden settings, where community transmission is now widespread. A large burden of DR-TB in Africa remains undiagnosed due to inadequate access to diagnostic tools that simultaneously detect tuberculosis and screen for resistance.

The identification of Mycobacterium tuberculosis as a causal agent of tuberculosis revolutionised the understanding of human diseases by showing that some have an infectious origin. How M tuberculosis spreads remained speculative until pioneering work—including having pulmonary tuberculosis patients cough directly onto guinea pigs—showed transfer of the bacillus to cause disease in a new host.1 This early link between symptomatic disease and cough-mediated M tuberculosis release established a concept that has informed tuberculosis science for decades—namely, that advanced lung pathology is a prerequisite for M tuberculosis transmission from a patient with tuberculosis.
