The condition is characterised by pain in the inguinal region and can lead to a significant number of absences from high-intensity physical activity. Osteoarthritis (OA) in the shoulder is a common problem in our patients. Thereby, in patients older than 60 years, a prevalence of up to 20% is mentioned [1,2]. However, due to the unique anatomy of the shoulder as a non-weight-bearing joint, surgical options such as arthroplasty have been traditionally used to a much lower extent, for example, for the hip and knee joints [3,4]. This is also based on the fact that the symptomatic glenohumeral osteoarthritis (GHOA) is not as common as osteoarthritis of the weight-bearing joints such as the hip and knee [1].

Arthroscopic Bankart repair (ABR) has been accepted as a standard procedure for anterior shoulder instability with a minimum or no glenoid bone loss and an on-track Hill-Sachs lesion if present. However, several controversies exist in the surgical treatment of anterior shoulder instability. This article will discuss some of these controversies in, “simple,” dislocations (without bone loss) as well as, “complex,” (with critical bone loss). Determining which patients will benefit from an arthroscopic procedure depends on multiple factors including age, activity level, adequate determination of bone loss, performed with feasible and reliable imaging techniques.

Primary repair of acute subscapularis (SSC) tears provides excellent results, but tendon retraction, muscle atrophy, fatty infiltration, and humeral head migration may render a more chronic tear irreparable. These irreparable SSC tears present a diagnostic and treatment challenge for orthopaedic surgeons. Careful physical examination and imaging evaluation can help to distinguish those with reparable versus irreparable tears, but they are still not very reliable due to the methodological limitations of current evidence.
