What's New in Cardiometabolic Health?

Dear readers,

Cardiometabolic health has become a discussion point in the presidential election, and many people in my preventive clinic are asking me if they have metabolic dysfunction. I think this is a good thing, and it opens the discussion on lifestyle, lab testing, and imaging results.

This collection focuses on cardiometabolic disease (CMD) and health (CMH). Anti-inflammatory diets are associated with a lower risk of dementia in older patients with CMD. Furthermore, does high fruit consumption worsen CMH? Not according to a new large study. What about extra virgin olive oil; is it certain it improves CMH? Not according to a recent review. According to a new study, Ferritin is not just a marker of iron storage but of inflammation and CMD. Do you want to switch from dairy to soy milk? Your CMH will not suffer.

In addition, a large study correlates meat consumption with the risk of developing type 2 diabetes. Intermittent fasting or reduced calorie testing in adolescents improved CMH. Unsurprisingly, poor CMH and low testosterone were identified. Finally, women with recent gestational diabetes are at higher risk for pre-diabetes, diabetes, and lower endothelial function.

    • We observed very small differences in adiposity and cardiometabolic biomarkers between those who reported high fruit consumption vs low, most of which did not persist over follow-up. Future studies on other mechanisms and detailed assessment of confounding might further elucidate the relevance of fruit to cardiovascular disease.

    • Having GDM within the past decade was associated with more adverse cardiac structure/function and vascular endothelial function. Some, but not all, risks may be mediated through the development of prediabetes or type 2 diabetes. Enhanced preventive efforts are needed to mitigate cardiovascular risk among women with GDM.
