Statin Use Associated With Reduced Risk of Dementia

Dear readers,

Heart disease is the #1 cause of death. Therefore, continued understanding of risk factors is crucial. This week, an analysis of dementia risk in users of statins reported a lower rate of dementia, particularly in the high-risk subset with apoE4 inheritance. Several articles on the genetic Lipoprotein(a) (Lp[a]) cholesterol demonstrate it is still rarely ordered routinely but is causal in atherosclerosis in millions of people; statins and fenofibrate raise Lp(a). One way to lower Lp(a) is through niacin. A provocative editorial comments on the value of over-the-counter niacin. I agree, and I use it for patients in my clinic as there are few alternative options.

In addition, apple cider vinegar received a thumbs up for improving metabolic measures like triglycerides, while meat intake was rated poorly in a study from Southern India. PCSK9 inhibitors were studied when starting in the hospital, a rare practice in the United States, and had efficacy, while another study failed to identify the benefit of these drugs on endothelial function when added to statins.

Be well,
Joel Kahn, MD, FACC

      • This randomized trial assessed the effect of alirocumab/statin vs. placebo/statin on flow mediated dilation (FMD) in acute myocardial infarction (AMI).

      • FMD improved throughout 1 year after AMI with medical therapy similarly in alirocumab/statin vs. placebo/statin treated patients.

      • FMD at baseline was significantly inversely associated with coronary plaque burden on intravascular ultrasound.
